https://healthplus.ai. Privacy is of great importance to Healthplus.ai. We therefore

comply with privacy regulation. This means that your data is safe with us and that we

will use it properly. In this privacy statement, we will explain what Healthplus.ai does

with the information we collect about you.

Please note that Healthplus.ai acts as a data processor in case it’s product is used by

healthcare providers such as hospitals. If you have any questions on the data use in

that context or want to exercise right you have under the applicable data protection

legislation, we kindly advise you to contact your healthcare provider, who is

responsible for the use of your data.

Healthplus.ai has appointed a Data Protection Officer (DPO). This person is

independent and monitors compliance of data protection legislation. The DPO can be

contacted by sending an email to dpo@healthplus.ai.

For what purpose do we process personal data?
Healthplus.ai collects personal data because we want to be of service to you as well

as possible. Your personal data will be processed by us:

• to enter into and execute contracts for services offered by Healthplus.ai

• to manage the relationships with Healthplus.a as a result of the contract

• to be able to contact and maintain contact with job applicants

• to send newsletters

We use personal data that you provide yourself or which we collect from you.

Performance of the contract with the customer

We will use your personal data to perform the contract with you (for example to

process assignments, invoice and communicate about our services).

For this purpose, we use your:

• Name and address details
• Email address.

• Phone number, if provided

• Invoicing and payment details

• Any other information necessary to perform the contract

We need this data to perform the contract with you. We store this information for as

long as necessary to perform the agreement and afterwards for as long as necessaryfor our administration and to comply with our retention periods. Certain types of

personal data will be retained for a longer period with regards to the legal tax

retention.

Contact

You can use our contact form to ask questions or make any request.

For this purpose, We use your:

• email address

We do this on the basis of legitimate interest to communicate with you. We store this

information until we are sure that you are satisfied with our response and for a

maximum of 2 years.

Job applicants

We process your personal data in order to assess your application and to carry out

our recruitment and selection process.

For this purpose, we use your

• name and address details

• email address

• phone number

• your resume and motivation letter

• information about your education, work experience and skills

• references and assessments results, if provided

• any other information you voluntarily share with us during the recruitment

process

We process your personal data on the basis of our legitimate interest in selecting a

suitable candidate for the vacancy. In this context, our interest is balanced against

your privacy rights.

We store this information for a period of 4 weeks after the end of the recruitment

process. If we wish to retain your application data beyond this period — for example,

to consider you for future vacancies — we will do so only with your explicit consent

and for a maximum period of 1 year, after which your data will be deleted. You may

withdraw your consent at any time.

Sending newsletters

We have a newsletter to inform those interested on news, tips and information about

our products and services. Your e-mail address is added to the list of subscribers,

only with your permission.

For this purpose, we use your:

• name
• email address.

We do this on the basis of your consent. We store this information until you cancel

your subscription. The newsletter is sent regularly by e-mail. You may cancel your

subscription to our newsletter at any time. Each newsletter contains a link to

unsubscribe from our newsletter.

Provision to other companies or institutions
We will only provide your personal data to other companies or institutions if this is

necessary for the execution of the contract, if we have obtained your consent to do

so, or if we are legally obliged to do so (for example, if the police demand this in the

event of a suspected crime). We work with certain companies. These are:

• External ICT service providers;

• Notified bodies/qualified entities for compliance purposes;

• The supplier of our administration tool and our administration office;

• Email and contact form service providers;

• The parties that supply cookies, see below.

How do we protect personal data?

Healthplus.ai takes the protection of your data seriously and takes appropriate

measures to prevent misuse, loss, unauthorized access, unwanted disclosure and

unauthorized modification. Healthplus.ai is ISO27001 certified and has implemented

a wide range of controls (e.g. encryption, 2-factor authentication, access control) to

ensure your information is securely transferred, processed and stored. lf you have the

impression that your details are not properly secured or there are indications of

abuse, or ways to improve our handling of data, please contact us at

Cookies

A cookie is a small text file that is stored on your computer, tablet or smartphone

when you first visit this website. Healthplus.ai uses technically necessary cookies.

These are cookies that are set by the site itself and can only be read by the site itself.

These cookies are used for functionality of the website. You can opt out of cookies

by setting your internet browser so that it does not store cookies anymore, but some

parts of our website may not work properly as a result of that. In addition, you can

also delete all information previously saved via the settings of your browser.

Framer Analytics

We use Framer Analytics to better understand how our website is used. This tool

allows us to collect general statistics about website visits, such as the number ofvisitors and the pages they view. We use this information to analyze, maintain, and

improve the performance and usability of our website.

Framer Analytics does not use cookies, and no directly identifiable personal data of

website visitors is processed through this tool.

Your privacy rights

You can always contact us if you have any questions regarding our privacy statement

withdraw your consent to the data processing or to object to the processing of your

personal data by Healthplus.ai. lf you wish to receive the (digital) personal data

processed by Healthplus.ai from you, you can rely on your right to data portability.

This means that you can submit a request to us to send the personal information we

have in our computer file to you or another organization mentioned by you.

You have the following rights:

• Right of access: you have the right to see what kind of personal data we

processed about you;

• Right of rectification: you have the right to rectify any personal data we

have processed about you, if this information is (partially) wrong;

• Right to complain: you have the right to file a complaint against the

processing of your personal data by us, or against direct marketing;

• Right to be forgotten: you can file a request with us to remove any personal

data we have processed of you;

• Right to data portability: if technically possible, you have the right to ask us

to transfer your processed personal data to a third party;

• Right to restriction of processing: you can file a request with us to

(temporarily) restrict the processing of your personal data.

If you exercise any of the rights mentioned above. We might ask for more information

to confirm your identity. If so, it is important that you hide your social security number

and photo. We will usually respond to your request within one month. This term can

be extended if the request is proven to be complex or tied to a specific right. You will

be notified about a possible extension of this term.

You can send your request based on the aforementioned rights or request for

cancellation of your consent or objection to the processing of your personal data

to privacy@healthplus.ai. We respond as quickly as possible, but within four weeks,

at your request.

Complaints

If you want to file a complaint about our use of personal data, please send an e-mail

with the details of your complaint to us. We will look into and respond to any

complaint we receive.

If you think that We are not helping you in the right way, you can file a compliant at

our DPO by sending an email to dpo@healthplus.ai.

After that, you have the right to file a complaint at the authority. For The Netherlands,

this is the Autoriteit Persoonsgegevens.

Changes to this privacy- and cookie statement

We reserve the right to modify this statement. We recommend that you consult this

statement on a regular basis, so that you remain informed of any changes.
